Hand it over at discharge
One page in the discharge folder, or the code on the after-visit summary. Nothing to set up for the nurse.

She joins when she’s ready
That night or three weeks later. The code tells Rebloom which clinic she came from and how many weeks she was, so week one reads right.

She chooses what you see
Check-ins, activity, her care network, escalations. Each is her call, and she can change it any time. Her Guide conversations are never shared.

Your team sees her on the panel
A weekly digest to the care coordinator, and a same-day page when the Guide hands her to a clinician.
